Each area is rated on a simple 1–5 scale, creating a quick visual picture of where the team is strong and where attention may be needed.

More Than Just A Score

The purpose of the Team Health Check is not to produce a perfect score.

The real value comes from the conversation behind the numbers.

For each dimension, the team is encouraged to capture evidence and observations.

Instead of simply saying:

“Our communication is a 3.”

Ask:

What are we seeing or experiencing that makes it a 3?

One team member may feel communication is working well while another sees important information arriving too late.

That difference in perspective is useful data.

How To Use The Team Health Check

Set aside around 30–45 minutes during a team meeting, workshop, retreat or project review.

Ask team members to first complete their ratings individually.

This matters because people are often influenced by the first opinion they hear.

Once everyone has reflected independently, compare the ratings as a group.

Look for areas where scores are similar, but pay particular attention to areas where perceptions are very different.

Then discuss the evidence behind the scores.

The objective is not to debate who is right.

The objective is to understand why people are experiencing the team differently.

The final section of the worksheet, From Insight To Action, helps the team identify:

Where would improvement make the biggest difference?

What will we do differently as a team?
